Before Your Activity

Every participant must complete the consent form before joining an Outdoor Addicts activity, tour, experience or transport service.

• Each adult participant should complete their own form.
• A parent or legal guardian must complete the form for minors.
• Please use the same name used for your booking.

What We Collect

The form collects basic contact details, emergency contact information, medical notes and your participant consent. This helps us manage safety and emergency preparation.
